Parece lógico que, de forma paralela al incremento progresivo de la capacidad científico, tecnológica y farmacológica de la medicina moderna, con el fin de modificar el curso y la llegada de la muerte, se haya desarrollado el debate acerca de los límites razonables para que el uso de dicha tecnología, siga sirviendo a los intereses de los individuos enfermos. En la actualidad ya nadie desconoce el término futilidad, pero en el ámbito médico y en el paciente critico irrecuperable o en fase terminal, los objetivos terapéuticos que definen el mismo se ven muchas veces confundidos. Entonces cuándo, cómo y bajo qué restricciones debe evitarse la instauración o el mantenimiento de determinadas medidas de soporte o de
tratamiento que puedan resultar inútiles o fútiles que no vienen a prolongar la vida, sino la agonía, el dolor o el sufrimiento, son las cuestiones en torno a las que se anuda lo que, con diferentes denominaciones,
se identifica hoy como Limitación del Esfuerzo Terapéutico (LET), que consiste en no aplicar medidas extraordinarias o desproporcionadas para la finalidad terapéutica que se plantea en un paciente con mal pronóstico vital y/o mala calidad de vida. Que puede ser de dos tipos: no iniciar determinadas medidas o retirarlas cuando están instauradas.Por tanto, la práctica de la LET parece estar presente en la medicina
de los últimos cincuenta años, aunque muchas veces no haya sido reconocida abiertamente, quizás por la falta de claridad conceptual, ética y jurídica que ha arrastrado hasta hace bien poco.
