A 74 -years old farmer from the Venezuelan Andes is living in a small village located at 3000 m of altitude. The patient had several skin nodules in his face and on the planta of his right hallux which had started to grow two years ago. There was a marked lack of sensibility of these lesions.

Dermatoyositis

A 27-years old patient from the State Portuguesa of Venezuela is presenting a swelling of his face and slightly elevated spots on both cheeks. In addition the patient had fever since 6 weeks, accompanied by myalgias in all members, articulations and the skeletal musculature.

Lepromatous Lepra

The nodule of the right halluz, mentioned in the fig. 41-A1, had a diameter of 3 cm. and presented ulceration.

Dermatomyositis

A biopsy of the skeletal musculature shows a non specific inflammation, necroses and hyalinosis of the muscle fibers. HE stain; 400 x.

The lymph liquid taken from the ear at the patient showed abundant red stained bacilli partly arranged in clusters. Ziehl-Neelsen stain; 1000 x.

Lepromatous Lepra

In a biopsy of the skin two typically pathologic skin nerves were observed in the HE stain. Especially the nerv on the right side was involved.
